Before going to US Club Soccer and the insurance directly, as a league we want to ensure it will be a claim that is eligible. It is also very important to determine that the player in question has been properly registered with the league through the proper channels and that their registration has been cleared with US Club Soccer.
If injured in a game be sure to have the referee make a note on the game report.

NOTES on Player registrations
Be sure to select the correct league/division/team when registering!
ALL players must complete the full registration before you are allowed to play. Payments must be made online. Clear face/mugshot pictures must be uploaded.
Player NAME and BIRTHDATE MUST BE EXACTLY as shown on whatever photo ID the player will use to confirm identity at all SoCal Premier league and cup games.
See Photo samples below to understand what is required. In sample1 the name is Alexander Joseph Sample, so if this player registers they must register with the name Alexander Joseph Sample. In sample 2 the name is Alexander J. Sample, if this player registers they must register with the name Alexander J. Sample.
We do not use player ID cards. Player check-in at games will be with a "Photo Gameday Roster". We do not require ID cards for check-in, however, if a referee cannot ID a player based on the photo they will ask for their photo ID, if the name or birthdate do not match with the roster then the player will not be allowed to play.
Player insurance has increased from $10,000 per player/per incident to $25,000. We felt it important to get more coverage for our players to help in any case of injury.
Athletes need to manage their SportsEngine accounts to ensure they get the most out of their SoCal Premier experience during each sporting season. With their accounts properly configured, athletes and their families will receive communications according to their preferences and be able to complete registrations more efficiently. In many instances, more than one person needs to be in the loop with a child's sports life. Using the mobile app, you can add additional guardians to an account. Guardians can 1) View games and events 2) RSVP to games and events 3) Send and receive messages to coaches and team members 4) participate in team chat
Once you've created your account, add your mobile phone so you can receive text messages from your team manager or coach.
Now that you have your mobile phone on your account, make sure you enable text messaging and any other notifications.
